化浊解毒中药联合标准三联疗法治疗幽门螺杆菌相关性慢性胃炎临床观察

目的观察化浊解毒中药联合标准三联疗法与枸橼酸铋钾颗粒+标准三联疗法的四联疗法2种不同方案治疗幽门螺杆菌相关性慢性胃炎(helicobacter pylori associated gastritis,HPAG)幽门螺杆菌(Hp)根除率、临床疗效、药物不良反应等,以探寻有效的HPAG治疗方案。方法将100例HPAG患者随机分为2组,对照组50例予标准三联疗法+枸橼酸铋钾颗粒治疗,治疗组50例予标准三联疗法+化浊解毒中药治疗。停止服药4周后复查14C尿素呼气试验(urea breath test,UBT),观察Hp的根除情况。釆用填写调查表的方式,分别于治疗前后对患者症状进行评分,通过治疗前后临床症状积分的变化来评价临床疗效,并比较治疗过程中不良反应发生情况。结果治疗组治疗后Hp根除率81.6%,对照组Hp根除率81.3%,2组比较差异无统计学意义(P0.05);治疗组总有效率95.9%,对照组总有效率66.7%,2组比较差异有统计学意义(P0.05),治疗组疗效优于对照组;治疗组不良反应发生率4.1%,对照组不良反应发生率25.0%,2组比较差异有统计学意义(P0.05),治疗组不良反应发生率低于对照组。结论 2种治疗方案在根除Hp方面效果相当,但化浊解毒中药联合标准三联疗法在提高临床疗效、改善临床症状方面优于含铋剂四联疗法,并能降低不良反应发生率。

Clinical observation of Huazhuo - jiedu Chinese medicine combined with standard triple therapy on helico-bacter pylori associated gastritis

Objective To observe the effects of Huazhuo - jiedu Chinese medicine combined with standard triple therapy or bi - containing quadruple therapy on Helicobacter pylori associated gastritis(HPAG),to observe the Hp eradication rate,clinical effects and adverse reaction and find out a effective therapy of HPAG. Methods 100 patients with HPAG were randomly divided into two groups. 50 cases in control group were treated by standard triple therapy combined with bismuth ptassium citrate granules,50 cases in treatment group were treated by standard triple therapy combined with Huazhuo - jiedu Chinese medicine. 4 weeks after cessation of therapy,the 14C urea breath test(UBT)was rechecked,and the Hp eradication was observed. The symptom before and after treatment were scored by questionnaire. The curative effect was evaluated by clinical symptoms score before and after treatment,and the occurrence of adverse reaction was compared between two groups. Results The Hp eradication rate after treat-ment in treatment and control group was 81. 6% and 81. 3% respectively,with no statistical differences(P > 0. 05). The total effective rate in treatment and control group was 95. 5% and 66. 7% respectively,with no statistical differ-ences(P > 0. 05),the clinical effect in treatment was superior to control group. The occurrence rate of adverse reac-tion in treatment and control group was 4. 1% and 25. 0%respectively,with statistical differences( P < 0. 05),the adverse reaction rate in treatment group was lower than con-trol group. Conclusion The effects of both therapy on e-radicate Hp are equivalent. But Huazhuo - jiedu Chinese medicine combined with standard triple therapy has better effects on increasing clinical effects and improving clinical symptoms than bi - containing quadruple therapy,which can decrease adverse reaction rate.
