分离上皮的直接免疫荧光检查诊断口腔天疱疮

目的：对口腔粘膜损害缘分离的上皮组织作直接免疫荧光检查（ Ｄ Ｉ Ｆ） ，探讨应用该技术诊断天疱疮的可靠性。方法：选择２２ 例口腔寻常型天疱疮，从患者口腔粘膜损害缘分离上皮，制成冰冻切片作为 Ｄ Ｉ Ｆ 的底物，用异硫氰酸荧光素（ Ｆ Ｉ Ｔ Ｃ）标记的兔抗人抗体染色检查，并与同一患者经传统活检取材的粘膜组织（ 含部分粘膜下层的完整粘膜块） 的 Ｄ Ｉ Ｆ 检查作比较。结果：分离上皮的 Ｄ Ｉ Ｆ，２２ 例全部有 Ｉｇ Ｇ 沉积在棘细胞间，并有３ 例 Ｉｇ Ａ、、１ 例 Ｉｇ Ｍ、１３ 例 Ｃ３ 沉积在棘细胞间，与传统活检法的粘膜组织 Ｄ Ｉ Ｆ检查结果相同。结论：从口腔粘膜损害缘分离的上皮组织作 Ｄ Ｉ Ｆ检查，用于诊断天疱疮其结果可靠，操作简便易行，创伤轻微。

Diagnosis of Oral Pem phigus by Direct Im munofluorescenceon Separated Epitheliu m

Objective : To determine whether direct im munoflurescence ( D I F) is reliable on the epithelial tissuesseparated from the edge of oral mucosal lesions for diagnosis of oral pemphigus . Methods : Twenty two patients with oralpemphigus vulgaris were selected . Frozen epithelial sheets ,separated from the edge of oral mucosal lesions in patients ,were used as a substrate ,staining with fluorescein isothiocyanate conjugated rabbit anti human antibody . The D I Ftests ofseparated epithelial sheets were compared with that oftwenty two conventional biopsy specimens , which is a sheet of oralmucosa including a part of submucosaltissues ,from the same patients . Results : In alltwenty two epithelial sheets ,in tercellular depositions of Ig G were evident. Ig A depositions were noted in three , Ig M in one and C3 in thirteen out ofthese epithelialsheets . The findings ofallconventional biopsy specimens on D I Fwere asthe same asthat of allseparatedepithelial sheets . Conclusion : Itis easy to perform and reliable to diagnote pemphigus with epithelial tissues from theedge of oral mucosallesions by D I Ftechnique .
