应用象鼻技术治疗主动脉疾病的疗效及随访观察

目的 :观察及评价象鼻技术治疗主动脉疾病的近期及中期疗效。方法 :1997年 12月至 2 0 0 2年 1月在全麻、深低温停循环加选择性脑灌注下行象鼻技术治疗主动脉疾病患者 2 6例。术后半年随访 18例。术后 1～ 3年间随访 19例。结果 :住院期间死亡 2例 ,脊髓并发症 2例。随访时死亡 1例 ,脑出血与脑梗塞各 1例。 2例行Ⅱ期胸主动脉置换及部分腹主动脉置换术。 1例行冠状动脉旁路移植术。结论 :象鼻技术是一种治疗广泛主动脉疾病安全有效的手术方法。大多数患者恢复良好 ,不需行Ⅱ期手术。

Evaluation and Follow-up of "Elephant Trunk Technique" in Aortic Surgery

Objective: To evaluate the effect of elephant trunk technique in the treatment of aortic disease through postoperative and follow up observation Methods: Twenty six patients received elephant trunk technique during 1997 to 2002 All operations were done under general anesthesia, deep hypothermic circulatory arrest and selective cerebral perfusion Eighteen patients were followed up 6 months after operation and 19 patients 1 3 years Results: There were 2 in hospital deaths and 2 spinal neurogenetic complications postoperatively There was one death in the follow up period, one cerebral hemorrhage and one stroke Two patients received second stage operation for replaced thoracic aorta One had coronary bypass surgery Conclusion: Elephant trunk technique is a safe, effective surgical procedure for extended aortic disease Most patients may recover well, with no requirement of further operation
