带锁髓内钉治疗新鲜四肢长骨干骨折临床分析

目的:探讨分析带锁髓内钉治疗新鲜四肢长骨干骨折的临床效果。方法:将我院112例新鲜四肢长骨干骨折的患者采用带锁髓内钉治疗的效果进行回顾性分析。结果:从手术情况分析:在院时间平均9.6 d。术前平均4.6 d;术中出血量平均为100 ml。112例中有106例骨折骨性愈合,愈合率为94.6%。骨折平均的愈合时间为16.4周,其中胫腓骨15周,股骨19周,肱骨12周。从术后的并发症情况看,骨折不愈合有9例,发生率为8.1%;术后感染有5例,发生率为4.4%;脂肪栓塞有2例,发生率为1.8%;断钉有12例,发生率为10.7%;邻近关节痛为16例,发生率为14.3%。结论:带锁髓内钉是治疗新鲜四肢长骨干骨折的理想方式之一,临床效果满意,但是需要注意其并发症。

Clinical analysis of the treatment of fresh limbs long bone fracture with interlocking intramedullary nail.

Objective To investigate and analyse the clinical effect of the treatment of fresh limbs long bone fracture with interlocking intramedullary nail.Method 112 cases of fresh long bone shaft fractures were retrospectively analyzed which were treated with interlocking intramedullary nail.Results From the analysis of the operation situation:average time was 9.6 d(5~14 d) in the experimental group and 4.6 d(3~8 d)in the preoperative group;intraoperative hemorrhage in experimental group was 100ml(70~220 ml),106 of 112 cases with fracture were healed,and healing rate was 94.6%,the average time of fracture healing was 16.4 weeks,the time of tibia and fibula was both 15 weeks,femur was 19 weeks and humeru was 12 weeks.From the postoperative complications situation,fracture nonunion was 9 cases and its incidence rate was 8.1%;postoperative infection was 5 cases and its incidence rate was 4.4%;fat embolism was 2 cases and its incidence rate was 1.8%;broken nail was 12 cases and its incidence was 10.7%;the adjacent joint pain happened and its incidence rate was 14.3%.Conclusion The treatment method of fresh limbs long bone fracture with interlocking intramedullary nail is an ideal way,the clinical effect is satisfactory,but need to be aware of its complications.
