高频振荡通气治疗新生儿呼吸窘迫综合征疗效分析

摘    要:目的:探讨高频振荡通气(HFOV)治疗新生儿呼吸窘迫综合征(NRDS)的疗效。方法:选取早产儿NRDS患儿98例,随机分为高频振荡通气治疗组和常频机械通气治疗组,比较两组之间的上机时间、上机后血气分析、氧合指数(OI)以及出现气胸等并发症发生率。结果:HFOV组上机后2 h、8 h、24 h的血气分析值和氧合指数明显优于CMV组,差异有统计学意义(P<0.05),气胸、慢性肺疾病等并发症的发生率HFOV组明显低于CMV组,差异有统计学意义(P<0.05)。结论:HFOV治疗早产儿NRDS较CMV能更好地改善氧合,更快地下调氧浓度,减少上机时间和肺部并发症。

Abstract:Objective To study the therapeutic effect of high frequency oscillatory ventilation on premature infants with respiratory distress syndrome(RDS).MethodWe assigned 98 infants with NRDS entry the study.They were randomly devided into high frequency oscillatory ventilation(HFOV) group and conventional mechanical ventilation(CMV) group.The blood gas analysis,time of breathing machine,oxygenation index and aeropleura were detected and compared between the two groups.ResultsThe HFOV group with the levels of the blood gas analysis,oxygenation index were higher than the CMV group(P<0.05).The attack rate of aeropleura,chronicity pulmonary disease in the HFOV group was lower than the CMV group(P<0.05).ConclusionIn the treatment of premature neonates with RDS,HFOV has a better pulmonary oxygenation and lower air leak than the CMV.
