下颌骨髁突骨折手术治疗的发展与现状

下颌骨髁突骨折的治疗一直存在着广泛的争议。近年来随着研究的深入和内固定技术的不断进步,手术治疗的适应证范围在不断扩大,疗效在不断提高,并发症的发生率也在不断降低;手术治疗越来越引起重视,传统的闭合性治疗受到了越来越严峻的挑战。下颌骨髁突骨折的手术需要遵循3个原则:即正确的解剖复位,可靠的固定和最小的损伤。作者就髁突骨折的分类、手术适应证、手术进路、固定方法以及术中术后的若干问题等作一综述。

Surgical treatment for mandibular condylar fractures:past, present and future

There has always been great controversy in the management of mandibular condylar fractures.Presently, with the progress of research and development of the technology of rigid internal fixation,more and more surgeons prefer surgical treatment to closed treatment. During operation 3 principles should be kept in mind: anatomic reduction ,stable fixation and minimally invasive surgery, so as to improve the prognosis and maximally restore TMJ function. This article reviews the surgical treatment for mandibular condylar fractures ,and is focused on the following points: fracture classification, surgical indications, surgical approaches, fixation methods, perioperative and postoperative management.
