单侧颞叶内侧癫痫伴海马硬化患者脑内在活动改变

摘    要:目的 观察颞叶内侧癫痫(mTLE)伴海马硬化患者脑内在活动(IBA)改变。方法 纳入38例单侧mTLE伴海马硬化患者(mTLE组)和53名健康志愿者健康对照(HC)组],比较组间IBA静态指标,包括低频振幅(ALFF)、比率ALFF(fALFF)、局部一致性(ReHo)、度中心性(DC)、全局信号相关性(GSCorr)和体素镜像同伦连接(VMHC)]及其对应动态指标dALFF、dfALFF、dReHo、dDC、dGSCorr及dVMHC,分析其与癫痫持续时间和认知评估量表的相关性。结果 相比HC组,mTLE组双侧丘脑、内侧颞叶、基底核、脑桥及小脑ALFF、dALFF和fALFF值,右侧扣带回及补充运动区fALFF值,右侧内侧颞叶、梭状回及基底核dfALFF值,脑桥、双侧丘脑、双侧尾状核及左侧内侧颞叶ReHo值均升高;左侧颞叶新皮质和额叶fALFF值和dfALFF值,双侧颞叶新皮质ReHo值和DC值,左侧颞叶新皮质dReHo和GSCorr值,双侧颞叶新皮质、小脑及内侧颞叶VMHC值,双侧枕叶、楔叶及补充运动区dVMHC值均降低。mTLE组形状连线测试A耗时与左侧丘脑ALFF值、左侧海马...

Abstract:Objective To observe changes of intrinsic brain activity (IBA) in mesial temporal lobe epilepsy (mTLE) patients complicated with hippocampal sclerosis. Methods Thirty-eight unilateral mTLE patients complicated with hippocampal sclerosis (mTLE group) and 53 healthy volunteers (group) were enrolled. IBA static indicators,including amplitude of low frequency fluctuation (ALFF), fractional ALFF (fALFF), regional homogeneity (ReHo), degree centrality (DC), global signal correlation (GSCorr) and voxel-mirrored homotopic connectivity (VMHC), as well as corresponding dynamic indicators, i.e. dALFF, dfALFF, dReHo, dDC, dGSCorr and dVMHC were observed and compared between groups, and the correlations of the above indicators and the duration of epilepsy and cognitive assessment scale were analyzed. Results Compared with those in HC group, ALFF, dALFF and fALFF values of bilateral thalamus, medial temporal lobe, basal ganglia, pons and cerebellum, fALFF values of the right cingulate gyrus and supplementary motor area, dfALFF values of the right medial temporal lobe, fusiform gyrus and basal ganglia, ReHo values of the pons, bilateral thalamus, bilateral caudate nucleus and left medial temporal lobe increased, while fALFF and dfALFF values of the left temporal lobe neocortex and frontal lobe, ReHo and DC values of bilateral temporal lobe neocortex, dReHo and GSCorr values of left temporal lobe neocortex, VMHC values of bilateral temporal lobe neocortex, cerebellum and medial temporal lobe, dVMHC value of bilateral occipital lobe, cuneiform lobe and supplementary motor area decreased in mTLE group. The duration of shape trail making test-A was positively correlated with ALFF value of left thalamus, fALFF value of left hippocampus and ReHo of the pons in mTLE group (r=0.577, 0.707, 0.489, all P<0.05), the duration of shape trail making test-B was positively correlated with dfALFF value of left pale ball and ReHo of the pons (r=0.701, 0.664; both P<0.05), but negatively correlated with VMHC value of cerebellum (r=-0.472, P=0.048). Conclusion Abnormal IBA dynamic and static indicators of multiple brain regions was found in unilateral mTLE patients complicated with hippocampal sclerosis, mainly distributed in temporal lobe epilepsy network related brain regions, temporal lobe neocortex, occipital lobe and cerebellum.
