综合护理在糖尿病患者胰岛素泵强化治疗中的应用价值

目的研究综合护理在糖尿病患者胰岛素泵强化治疗中的应用价值。方法住院治疗的糖尿病患者140例，按照抽签的方式将患者随机分为观察组和对照组各70例。对照组给予常规护理，观察组给予综合护理，比较二组患者的血糖控制情况、生存质量、心理健康状况及不良事件发生情况。结果观察组患者的空腹血糖、餐后2h血糖、血糖达标时间、胰岛素用量及生存质量情况均优于对照组患者的控制情况，且组间比较差异有统计学意义（P〈0.05）。观察组患者SAS量表明显低于对照组（P〈0.05），对照组存在一定的焦虑症状；而SDS量表检测发现二组间差异无统计学意义（P〉0.05）。出现低血糖患者中，观察组3例，对照组12例，组间比较差异有统计学意义（X^2=6.048，P〈0.05）。观察组出现皮肤感染1例，显著低于对照组7例，差异有统计学意义（X^2=4.773，P〈0.05）。结论综合护理在糖尿病患者行胰岛素泵强化治疗过程中能有效地控制血糖，减少不良事件的发生，提高患者的生存质量，具有十分重要的临床和社会价值。

THE VALUE OF COMPREHENSIVE CARE IN PATIENTS WITH DIABETES BY INSULIN PUMP STRENGTHEN THERAPY

Objective To explore the value of comprehensive care in patients with diabetes treated by intensive insulin pump therapy. Methods Totally 140 cases of diabetes who hospitalized in the endocrinology department from August 2012 to August 2013 were observed, who were randomly divided into experimental group and control group, 70 cases in each group. The control group received routine care and the experimental group received comprehensive care. Then blood glucose control, quality of life, mental health and the incidence of adverse events of two groups were compared. Results Fasting glucose, 2 - hour postprandial blood glucose, blood glucose time, insulin dosage and quality of life in experimental group were superior to the situation of control group, the difference between two groups had statistical significance （P〈0.05）. The result of SAS scale in experimental group was significantly lower than the control group （P〈 0.05）, there was a certain anxiety symptoms in the control group; while SDS scale testing found no significant difference between the two groups （P〈0.05）. Three cases of patients with hypoglycemia were found in the experimental group and 12 cases in the control group, the difference between the two groups had sta- tistical significance （X^2 = 6. 048,P〈0. 05）. The experimental group had one case of skin infections, it was significantly lower than the control group（7 cases）, the difference between the two groups had statistical significance（X^2 = 4. 773,P〈0.05）. Conclusion Integrated care can effectively control blood sugar, reduce the incidence of adverse events and improve the quality of life of patients with clinical and social value in the process of the treatment of diabetes patients with intensive insulin pump.
