经尿道电汽化术治疗膀胱癌

目的探讨经尿道膀胱肿瘤电汽化术( TVBt )治疗浅表性膀胱移行细胞癌的疗效及其安全性.方法对 213 例浅表性膀胱癌患者行 TVBt 术.并发良性前列腺增生症( BPH )者同时行经尿道前列腺电汽化术( TUVP ).术后采用丝裂霉素及卡介苗膀胱灌注.结果 213 例膀胱肿瘤均为 1 次切除,平均手术时间 TVBt 为 20 min , TVBT 加 TUVP 为 65 min .术中无膀胱穿孔,全组均无输血.术后平均留置尿管约 6 d. 210 例获随访,随访时间 0.5-8 年,复发 56 例( 27% ),其复发率与同期膀胱部分切除术相比差异无统计学意义.结论 TVBT 操作简单、损伤小、出血少、恢复快、疗效好、可重复手术.

Transurethral vaporization treatment of bladder carcinoma-report of 213 cases

Objective To approach the therapeutic effects and safety of the operation of transurethral vaporization of bladder tumor(TVBt) for the treatment of superficial transitional cell carcinoma of bladder.Methods TVBt was performed in 213 cases of superficial bladder carcinoma.For the patients complicated with henign prostate hyperplasia(BPH) transurethral vaporization prostatectomy(TUVP) was performed simultaneously.After the operation,mitomycin and BCG were used for the Irrigation of bladder.Results All bladder tumors of 213 cases were removed with one time operation,the average period of the operation was 20 minutes for TVBt and 65 minutes for TVBt plus TUVP.There was no vesical perforation and blood transfusion during the operation.Urethral catheter was kept about 6 days after the operation.Among the 213 cases,210 cases were followed up from half a year to 8 years.Relapse was found in 56 cases(27%) and the ratio of relapse was not statistically significant compared with that of partial bladder resection.Conclusion The operation of TVBT is simple with small injury and less bleeding.The patients undergoing TVBT show better recovery.It is an ideal therapy for the treatment of superficial transitional cell carcinoma of bladder.
