可达龙坎地沙坦联合治疗冠心病阵发性房颤的疗效观察

目的 了解可达龙（胺碘酮）坎地沙坦联合治疗冠心病阵发性房颤的疗效.方法 选择我院2006年1月至2009年2月冠心病阵发性房颤患者60例,随机分为可达龙组（对照组）和可达龙坎地沙坦联合治疗组各30例,总疗程48周,观察两组左房内径变化和房颤复发病例数.结果 可达龙组心房颤动患者治疗后复发18例,联合治疗组复发10例.两组心房颤动的转复率差异有统计学意义（P〈0.05）,联合治疗组左房内径降低数明显优于对照组（P〈0.05）.结论 可达龙联合坎地沙坦对阵发性心房颤动的复律和复律后窦性心律维持,以及左房内径影响均优于可达龙单用治疗,可能与坎地沙坦抑制肾素-血管紧张素系统、降低心脏负荷、抑制心房电及结构重构有关.

Objection on efficacy of cordarone combined with Candesartan to treating paroxysmal atrial fibrillation

Objective To realize the efficacy of cordarone （amiodarone） combined with Candesartan to treating paroxysmal atrial fibrillation. Methods To choose 60 patients who have paroxysmal atrial fibrillation and hospitalized in our hospital from January in 2006 to February in 2009, and were randomly divided in to two groups： 30 cases belong to the treat group （cordarone combined with Candesartan group）and the other 30 eases belong to the control group （eordarone group）. After the total course （48 weeks ） of treatment, investigating the change of internal diameter. And the cases of recurrence. Results The cases who recurred were 18 in the control group and 10 in the treat group. The rate of cardioversion of two groups is maked different （P〈0.05）.Combined treatment group is superior to the control group in changing the internal diameter of the left atrium （P〈0.05）. Conclusion The group is superior to the control group in cardioversion of Paroxysmal atrial fibrillation, in keeping the rhythm of the heart and in infecting left atrium diameter. It maybe have some relationships with candesartan＇s inhibiting the rennin-angiotensin system, Reducing the cardiac load,suppressing atrial eletrial remodeling and atrial structural remodeling.
