| 摘 要： | 目的介绍一种改良的经膈肌胸、腹膜外胸腹联合切口治疗胸腰段脊柱病变。方法回顾性分析88例胸腰段病变的患者,均采用切除第11肋的经膈肌胸、腹膜外胸腹联合切口,直接暴露胸腰段椎体,施行各类脊柱手术。结果本组手术暴露时间为30~50 m in,病灶清除彻底,螺钉位置理想。术后发生气胸1例,乳糜液漏1例。62例平均随访23个月,无钢板断裂、植骨不融合、脊柱后凸畸形等并发症发生。结论该手术切口对组织损伤小,手术视野好,术后并发症少,适用于胸腰段脊椎病变的手术。

| Abstract: | Objective To introduce an extraperitoneal thoraco-abdominal approach for thoracolumbar spine via diaphragm with the removal of 11 rib.Method Extraperitoneal thoraco-abdominal approach for thoracolumbar spine via diaphragm with the removal of 11 rib was utilized for 88 cases of thoracolumbar spine,the operating field was exposed well.Result Operative exposure duration was 30 to 50 minutes,there was 1 case of pneumothorax and 1 case of chylous leak.There was no complication such as plate abruption,nonunion,hyphosis in 62 cases within average 23 months follow-up.Conclusion The approach serves the surgery for thoracolumbar spine well with less invasion and better exposure. |
