米索前列醇联合缩宫素预防产后出血的疗效探讨

目的探讨米索前列醇与缩宫素联用在产后出血预防方面的临床效果。方法选取2013年5月—2014年5月该院收治的70例自然分娩产妇,随机将其均分为对照组与实验组各35例,娩出胎儿后对照组给予产妇10IU缩宫素静注,实验组在此基础上给予米索前列醇片经舌下含化。观察两组产妇的疗效。结果实验组第三产程（7.4±2.6）min,明显短于对照组（15.4±4.0）min,差异有统计学意义（P〈0.05）;实验组产后出血率2.9%,明显少于对照组17.1%,差异有统计学意义（P〈0.05）;两组不良反应、脉搏和血压变化比较,差异无统计学意义（P〉0.05）。结论在产后出血预防过程中,联合应用缩宫素和米索前列醇安全有效,可显著降低产后出血率及出血量,安全性高,值得推广应用。

Investigation of the Efficacy of Misoprostol Combined with Oxytocin for the Prevention of Postpartum Hemorrhage

Objective To investigate the clinical effect of misoprostol combined with oxytocin on preventing postpartum hemorrhage. Methods 70 puerperants underwent natural labor in our hospital from May 2013 to May 2014 were selected and randomly and equally divided into the control group and the experimental group with 35 cases in each. After the fetus was delivered, the puerperants in the control group were given intravenous injection of oxytoein 10IU, and the puerperants in the experimental group were given sublingual administration of misoprostol tablets based on the treatment given to the eontrol group. And the efficacy of the two groups were observed. Results The third stage of labor of the experimental group was （7.4±2.6） min, obviously shorter than the control group＇s （15.4±4.0） min （P〈0.05）; the postpartum hemorrhage rate of the experimental group was 2.9%, significantly less than the control＇ group＇s 17.1%（P〈0.05）; there was no significant differences in adverse reactions, pulse and blood pressure changes between the two groups （P〉0.05）. Conclusion For preventing the postpartum hemorrhage, oxytocin combined with misoprostol can significantly reduce the rate and amount of postpartum hemorrhage with high safety and effectiveness, so it is worthy of promotion and application.
