三种方法联合治疗青光眼合并白内障的疗效探讨

目的研究白内障超声乳化联合小梁切除术联合人工晶状体植入术（后称三联疗法）治疗青光眼合并白内障的临床效果。方法选择2011年2月—2013年6月该院收治的109例青光眼合并白内障患者进行回顾性研究,所有患者均采用三联疗法治疗,治疗后,对其疗效进行比较分析。结果治疗后,所有患者眼压及裸眼视力较治疗前均显著改善,差异有统计学意义（P〈0.05）;急性组治疗后眼压改善情况显著优于慢性组,差异有统计学意义（P〈0.05）;急性组与慢性组手术成功率均较高,分别为93.1%、80.4%,差异有统计学意义（χ2=3.9170,P=0.0478）。结论采用白内障超声乳化联合小梁切除术联合人工晶状体植入术治疗青光眼合并白内障效果显著,尤其针对急性闭角型青光眼合并白内障,效果更佳。

Observation on the Combined Treatment Effect of Three Methods on Glau- coma Complicated by Cataract

Objective To study the clinical results of phacoemulsification and trabeculectomy combined with intraocular lens im- plantation（hereinafter referred to as triple therapy） in the treatment of glaucoma and cataract. Methods 109 patients with glaucoma complicated by cataract admitted in our hospital from February 2011 to June 2013 were selected for the retrospective study. All the patients were treated by the triple therapy. After treatment, the treatment effect was analyzed comparatively. Results After treatment, the intraocular pressure and naked visual acuity of all the patients improved significantly compared with those before treatment （all P〈0.05）; the improvement in intraocular pressure of the acute group was obviously better than that of the chronic group （P〈0.05）; the surgical success rate in both groups was high, which was 93.1%, 80.4% in the acute group and chronic group, respectively, and the difference between the groups was statistically significant （χ2=3.9170, P=0.0478）. Conclusion In summary, phacoemulsification and trabecnlectomy combined with intraocular lens implantation have remarkable effect on glaucoma compli- cated by cataract, and the effect will be better especially on acute angle-closure glaucoma complicated by cataract.
