内固定联合封闭负压吸引治疗开放性胫腓骨骨折的疗效探讨

目的探讨内固定术联合封闭负压吸引（VSD）技术治疗开放性胫腓骨骨折的临床治疗效果。方法将2012年3月—2014年2月该院收治的72例开放性胫腓骨骨折患者,随机分为两组,每组36例。试验组采用内固定联合负压封闭吸引（VSD）技术进行治疗,二期修复创面;对照组采用传统的I期清创、外固定、术后创面换药,二期手术内固定及创面修复进行治疗,比较两组患者的临床治疗效果。结果试验组患者具有较好的创面修复能力,骨折愈合良好;与对照组患者相比,试验组患者的住院治疗时间及II次创面修复时间都显著缩短,降低了创面感染的发生率,两组差异有统计学意义（P〈0.05）。结论对开放性胫腓骨骨折患者采用内固定联合负压吸引（VSD）技术治疗具有显著的临床疗效,可有效地缩短住院时间,大大地降低感染率,减轻患者的痛苦,值得在临床上推广使用。

Observation on the Curative Effects of Internal Fixation Combined with Vac-uum Sealing Drainage in the Treatment of Compound Tibiofibula Fracture

Objective To observe the clinical effect of internal fixation combined with vacuum sealing drainage（VSD） on compound tibiofibula fracture. Methods 72 cases of patients with compound tibiofibula fracture admitted in our hospital from March2012 to February 2014 were selected and randomly divided into two groups, 36 cases in each group. The test group were treated by internal fixation combined with VSD, repairing the wound in the two-stage operation, and the control group were treated by the traditional method, such as Phase I debridement, external fixation, postoperative wound dressing, internal fixation and repairing the wound in the two-stage operation. And the clinical treatment effects were compared between the two groups. Results For the test group, all the wound surface and fracture healed well, compared with the patients in the control group, patients in the test group had less length of stay and wound healing time in the two-stage operation, which reduced the incidence of wound infection, the difference between the two groups was statistically significant（P〈0.05）. Conclusion Internal fixation combined with VSD for compound tibiofibula fracture is of great clinical value, which can effectively shorten the length of stay, reduce the wound infection rate significantly, alleviate the suffering of patients, so it is worthy of clinical promotion and application.
