不同浓度A型肉毒毒素治疗痉挛型脑瘫尖足的临床探讨

目的为探索A型肉毒毒素治疗小儿中枢性痉挛的最佳浓度。方法选择2011年7月—2013年7月在该科收治的小儿脑瘫尖足患儿46例,随机分为A、B两组,采用兰洲生物制品产生产的A型肉毒毒素衡力配制成两种不同浓度,A组浓度为50 u/m L;B组浓度为25 u/m L。用肌电引导仪的电刺激模式定位注射患儿小腿三头肌。结果 A、B两组在起效时间、疗效达高峰时间、疗效持续时间、小腿三头肌痉挛的缓解程度方面差异无统计学意义。结论用A型肉毒毒素50 u/m L和25 u/m L的浓度注射小腿三头肌,在治疗小儿中枢性痉挛所致的尖足症状上,疗效确切,效果相近。

Clinical Observation on the Effect of Different Concentration of Botulinum Toxin Type A in the Treatment of Spastic Cerebral Plsy Equinus Foot

Objective To explore the optimal concentration of botulinum toxin type A for the treatment of infantile cerebral spasm.Methods 46 children with spastic cerebral plsy equinus foot admitted in our department from July 2011 to July 2013 were randomly divided into two groups, group A and group B. Solutions of 50u/ml and 25u/ml were prepared from Hengli brand Botulinum toxin type A produced by Lanzhou Biological Product Factory and injected locatedly to the triceps surae of group A and group B, respectively, by the electrical stimulation mode of electromyogram guided instrument. And the degrees of spasm before and several time quantums after the injection were evaluated. Results There were no significant differences in the onset time, the time for the effect reaching peak, the duration of the response and the relieving degree of triceps muscle spasm between the two groups. Conclusion Solutions of botulinum toxin type A with 50u/ml and 25u/ml injected to the triceps surae for the treatment of spastic cerebral plsy equinus foot have exact effect and the effects of the two solutions are similar.
