硬膜外血肿手术治疗结果相关因素分析

目的总结硬膜外血肿的治疗情况，分析预后相关因素。方法对2002—09～2004—07北京天坛医院成人外伤病房急诊手术治疗的173例硬膜外血肿病人的资料收集整理，进行统计分析。结果术后结果与以下因素相关，包括病人的年龄、是否合并损伤、血肿量、术前意识、GCS评分、瞳孔、脉搏、呼吸、血压以及术中出血量。最有意义的预测指标包括是否双瞳散大、是否合并颅内其他损伤、年龄。结论手术应尽量赶在双瞳散大之前进行。年龄大的或伴有脑挫伤或硬膜下出血的病人更应给予关注。

Analysis of the surgical treatment outcome-related factors for epidural hematoma

Objective To summarize the effect of surgical treatment on epidural hematoma and analyze the prognosis-related factors. Methods To collect and analyse clinical informations of the 173 patients admitted to Beijing Tiantan hospital from Sep 2002 to Jul 2004,and given operation. Results The post-operation outcome was relative to the patient's age,the multiple injury,pre-operation consciousness level,GCS level,the size of the pupil, pulse rate,respiratory rate,blood pressure and the hemorrhage volume during the operation.The most important prognostic indexes included the patient's age,the size of the pupil, with or without complicated injury etc. Conclusions The operation should be given before the both pupils enlarge. The patients are old or have cerebral contusion or subdural hematoma should be paid close attention.
