全部胃小弯切除在远侧胃癌根治术中的应用

目的:探讨全部胃小弯切除在远侧胃癌根治术中的疗效。方法:对32例远侧胃癌在进行D₂胃癌根治术的同时,施行全部胃小弯切除的临床资料,对术后并发症,所切除的胃小弯标本中癌细胞残留情况,术后生存率等进行分析。结果:围手术期无病死病例,术后病理检查显示,所切除的胃小弯标本中癌细胞残留率15.6%。所清扫的淋巴结中,贲门右淋巴结阳性率21.9%。术后1、2、3年生存率分别为90.6%、78.1%、65.6%。结论:在远侧胃癌根治术中应用全部胃小弯切除,能够更彻底的清除转移淋巴结,避免胃小弯切缘可能存在的癌细胞残留,提高根治性切除率,降低复发率,提高术后生存率。
