肝脏占位性病变的低场MRI应用

摘    要:目的探讨低场MRI在肝脏占位病变中的鉴别诊断价值。方法应用0.2T永磁型磁共振仪,对6个CT不能明确诊断的肝脏占位性病变行T1WI、T2WI平扫,部分再使用快速多层面扰相位梯度回波(FMPSPGR)序列作进一步同层块动态增强扫描。磁共振造影剂为磁显葡胺(Gd-DTPA)0.2mmol/kg。结果 2例有脂肪肝背景的肝细胞肝癌、1例无明显胆管扩张的肝胆管细胞癌和3例肝局灶性结节增生(focal nodular hyperplasia,FNH)均作出明确诊断。结论低场MRI在肝脏占位病变鉴别诊断中,对有脂肪肝背景的肝细胞肝癌,胆管扩张不明显的胆管细胞癌和肝FNH有较高临床应用价值。

Abstract:Objective To investigate the value of diagnosis of hepatic lesions in low-field MRI.Methods 6 patients with unknown focal liver lesion were scanned by 0.2T MRI.All cases were unable to be definitely diagnosed after CT inspection.All patients were scanned with SE sequence T1WI and T2WI at first.Part of them were imaged with FMPSPGR for further dynamic contrast enhancement at the same-level.The contrast medium for magnetic resonance is Gd-DTPA(0.2mmol/kg).Results Definite diagnosis confirmed by pathology were given for all cases as follows: 2 cases of hepatic cell cancer with fatty livers;1 case of cholangiocarcinoma without obvious expansion of bile duct;3 cases of liver FNH.Conclusion Low-field MRI is helpful for identification and diagnosis of such cases as hepatic cell cancer with fatty liver,cholangiocarcinoma without obvious expansion of bile duct and liver FNH.
