冻存羊膜移植并糖皮质激素治疗眼表重度碱烧伤效果

目的 探讨冻存羊膜移植联合应用糖皮质激素对早期重度眼表碱烧伤的疗效。方法 将重度眼表碱烧伤病人34例（38只眼）分为两组，手术组28眼在早期应用糖皮质激素的基础上行冻存羊膜移植术，对照组10眼早期应用糖皮质激素。结果 随访3～12个月，手术组所有病眼术后视力均有不同程度提高，20眼角膜水肿减轻，21眼角膜遗留不同程度的斑翳，4眼角膜恢复透明，1眼持续混浊、水肿并发生角膜溃疡，2眼发生睑球粘连。对照组3眼角膜溃疡，6眼角膜白斑伴角膜血管化，4眼发生不同程度睑球粘连。结论 早期羊膜移植并糖皮质激素治疗严重眼表碱烧伤具有较好的效果。

A CLINICAL OBSERVATION OF COMBINATION OF CRYOPRESERVED AMNIOTIC MEMBRANE TRANSPLANTATION WITH GLUCOCORTICOID FOR SEVERE ALKALI BURNS OF CORNEA

Objective To assess the curative effect of cryopreserved amniotic membrane transplantation combined with glucocorticoid for severe alkali burns of cornea at early stage. Methods Thirty-eight eyes(34 patients) with severe alkali burns of cornea were divided randomly into two groups: transplantation group(28 eyes),received cryopreserved amniotic membrane transplantation on the bases of glucocorticoid therapy,and control group(10 eyes),received glucocorticoid therapy alone at early stage of burns. Results A follow-up of 3 to 12 months showed that in the transplantation group,the visual acuity of all the patients improved to various extent.Corneal edema in 20 eyes reduced,four corneas became transparent.But,macular nebula still remained in 21 corneas with one being dropsy and even ulcerous. Atretoblepharia was found in two eyes.In the control group,ke-ratohelcosis was found in three eyes,big keratoleukoma with corneal vasculation in six and atretoblepharia in different degrees in four. Conclusion On the basis of glucocorticoid therapy,amniotic membrane transplantation at early stage will produce good effect on the serious alkali burns of cornea.
