绝经后妇女2型糖尿病性激素变化与冠心病并发症关系

摘    要:目的 探讨绝经后妇女2型糖尿病患者血清性激素水平变化与冠心病并发症关系。方法 测定25例绝经后妇女2型糖尿病并发冠心病(A组)、23例2型糖尿病未并发冠心病(B组)、20例绝经后正常妇女(C组)的血清雌二醇(E2)、孕激素(P)、促卵泡刺激素(FSH)、促黄体生成素(LH)、睾酮(T);同时测定了血浆总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)及低密度脂蛋白胆固醇(LDL-C)。结果 A组与B组、C组比较显示、E2、HDL-C水平显著降低。P、FSH、LH、TC、LDL-C水平显著半高,线性相关分析显示E2与FSH、LH、TC、TG、LDL-C呈显著负相关,而与HDL-C呈显著正相关。结论 绝经后妇女2型糖尿病合并冠心病患者存在着严重的性激素失调及脂代谢异常,可能为绝经后妇女2型糖尿病并发症冠心病的原因之一。

Abstract:Objective To study the changes of sex hormone in postmenopausal women with type 2 diabetes mellitus and the relationship between sex hormones and coronary heart disease complication.Methods 25 postmenopausal women with type 2 diabetes mellitus complicated by coronary heart disease(group A),23 postmenopausal women with type 2 diabetes mellitus and no coronary heart disease complications(group B) and 20 postmenopausal women without diabetes mellitus as control(group C) were selected to compare the serum levels of estradiols (E 2),progesterone(P),follicle stimulating hormone(FSH),luteinizing hormoe(LH),testosterone(T),total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL C) and low density lipoprotein cholesterol(LDL C).Results Analysis of variance revealed that serum levels of P,FSH,LH,TC,LDL C were significantly higher while E 2 and HDL C lower in group A than those in group B and group C.Correlation analysis showed that serum E 2 level was negatively correlated with serum FSH,LH,TC,TG and LDL C levels,but positively with HDL C.Conclusion The study indicates that the endogenous sex hormones disorder and adverse serum lipid profile play an important role in the development of type 2 diabetes mellitus and coronary heart disease complication.
