多层螺旋CT对飞行员肺栓塞诊断价值的研究

摘    要:目的:探讨多层螺旋CT(multislice spiral computed tomography,MSCT)对飞行员肺栓塞(pulmonary embolism,PE)的诊断价值。方法:对15例患者行MSCT扫描及最大密度投影(MIP)、多平面重建(MPR)、容积重建(VR)等图像处理操作,观察栓子的形态、大小及位置,并分析MSCT对PE的诊断结果。结果:15例疑似PE的患者中,MSCT确诊7例,共46支肺动脉发生血栓栓塞,其中肺动脉干、肺叶动脉、肺段动脉、肺亚段动脉、5级肺动脉发生血栓栓塞的比例分别为6.52%、26.09%、45.56%、19.57%及2.17%。结论:MSCT对飞行员PE患者的诊断具有重要价值,能准确、清楚地显示栓子的形态、大小、位置及其他间接征象。

Clinical evaluation of multi-slice spiral computed tomography in diagnosis of pulmonary embolism of pilots

Abstract:Objective To evaluate the clinical value of multi-slice spiral computed tomography (MSCT) for the diagnosis of pulmonary embolism (PE) of the pilots. Methods Totally 15 pilots suspected with PE underwent maximum intensity projection (MIP), multi-planar reconstruction (MPR), volume reconstruction (VR) and etc, and then were analyzed retrospectively for MSCT data. The morphology, size and place of the embolus were observed, and the value of MSCT for PE was analyzed. Results Seven patients were confirmed by MSCT, with 46 pulmonary arteries embolized. The percents for embolism in pulmonary trunk, lobar artery, segmental artery, subsegmental artery and the fifth pulmonary artery were 6.52%, 26.09%, 45.56%, 19.57% and 2.17% respectively. Conclusion MSCT is valuable for the diagnosis of the pilot PE, which can clearly display the morphology, size, place and indirect signs of the embolus.
