肺功能异常患者发声功能的空气动力学研究

摘    要:目的观察肺功能异常患者嗓音空气动力学变化特征,研究通气功能障碍对嗓音空气动力学的影响,探讨呼吸功能与发声功能之间的关系。方法采用空气动力学测试系统分别对50例肺功能异常患者和50例健康成人的嗓音持续性元音信号/a/进行测试,观察并分析其真声最低音、舒适音调和真声最高音时空气动力学参数(最长发声时间和平均气流率)。所有病例按性别分为两组,比较不同频率正常人与肺功能异常患者发音各参数的改变,并观察肺功能障碍程度对各参数的影响。结果肺功能异常患者和健康成人在真声最低音时空气动力学参数结果差异有统计学意义(P<0.05或0.01);在舒适音调和真声最高音时,肺功能异常患者发声时间与健康成人相比明显缩短(P均<0.05)。舒适发声方式下,各参数与肺功能严重程度有明显的相关性。结论肺功能异常在三种音调发声时对嗓音空气动力学均产生一定影响;参数变化与肺功能严重程度呈正相关。呼吸功能与发声功能之间存在一定关系。

Abstract:Objective To investigate the areo-dynamic features of voice in patients with abmormal lung function,and to evaluate the effect of lung disfunction on areo-dynamics of voice.The relationship between respiration and phonation will be discussed.Methods The voice signals of sustained vowel /a/ were measured by a micro-computer with Dr Speech science for windows software and an aero-dynamic system in 50 patients with abmormal lung function and 50 healthy adults. The areo-dynamic parameters(mean airflow rate,MPT) were collected and analysed in the lowest pitch,normal pitch and the highest pitch.All patients were divided into two groups according to gender.Results In the patients of abmormal lung function ,the aero-dynamic parameters (MPT, mean air flow rate) were significantly decreased in the lowest pitch. In normal and the highest pitch, MPT were decreased significantly, other parameters changed too,but had no statistic value. There was a positive relationship between the degrees of lung disfunction and the areo-dynamics parameters.Conclusion Abnormal lung function affect significantly on areo-dynamics of voice in three different pitches.The changes of the parameters are related to the degrees of lung function.The results show that respiratory activities play an important role in phonation.
