PCNA和pRb2/p130在子宫内膜癌中的表达及临床意义

目的：探讨增殖细胞抗原（ PCNA）和pRb2/p130在正常子宫内膜、子宫内膜增殖症、非典型子宫内膜增殖症和子宫内膜癌中的表达、相关性及与临床病理特征之间的关系。方法：免疫组织化学EnVision二步法检测PCNA和pRb2/p130在30例正常增生期子宫内膜、30例子宫内膜增殖症、40例非典型增生子宫内膜增殖症和76例子宫内膜癌中的表达。结果：PCNA在非典型子宫内膜增殖症、子宫内膜癌中的表达分别高于正常子宫内膜和子宫内膜增殖症，差异显著（ P＝0.043；P＝0.020），pRb2/p130的表达分别低于正常子宫内膜和子宫内膜增殖症，差异显著（ P﹤0.05）；子宫内膜癌中，PCNA与肿瘤大小、分期、淋巴结转移和雌激素差异显著（P﹤0.05），PCNA在子宫内膜癌中的表达显著高于子宫内膜增殖症（P＝0.045），pRb2/p130与年龄、组织学分级、浸润深度、淋巴结转移和雌激素有差异（ P﹤0.05），pRb2/p130和PCNA呈负相关（ r＝-0.331，P＝0.003）。结论：pRb2/p130和PCNA在非典型子宫内膜增殖症和子宫内膜癌中的异常表达，结合在一起可能成为评估识别具有高风险的子宫内膜癌患者的新参数。

The expression and clinical significance of PCNA and pRb 2/p 1 3 0 in endometrial carcinoma

Objective:To investigate the expressions of PCNA and pRb2/p130,their correlation and relationship with the pathological clinical features of normal endometrial tissue,endometrial hyperplasia,atypical endometrial hy-perplasia and human endometrial carcinoma. Methods:Thirty normal endometrial tissue,30 endometrial hyperplasia, 40 atypical endometrial hyperplasia and 76 endometrial carcinomas were collected. Immunohistochemistrical methods were used to detect pRb2/p130 and PCNA expressions. Results:In the atypical endometrial hyperplasia and endome-trial carcinomas,the expressions of PCNA were significantly higher than that of normal and endometrial hyperplasia( P=0. 043;P=0. 020,respectively),but the expression of pRb2/p130 was significantly lower than that of normal and endometrial hyperplasia. In the endometrial carcinomas,the expression of PCNA was significantly related tumor grade, staging,lymph node metastasis and ER respectively(P﹤0. 05). The expressions of PCNA were significantly higher than that of endometrial hyperplasia(P=0. 045). The expression of pRb2/p130 was significantly realted with age, histopathologic types,invasion depth,lymph node metastasis and ER respectively(P﹤0. 05). There was negative cor-relation between the expressions of pRb2/p130 protein and that of PCNA(r= -0. 331,P=0. 003). Conclusion:The abnormal expression of pRb2/p130,and PCNA in the atypical endometrial hyperplasia and that of endometrial carci-nomas,combined together,could be considered new parameter to be evaluated in discriminating patients at higher risk for endometrial carcinoma.
