15例脐尿管病变的诊治分析

目的:对脐尿管肿瘤做到早发现早治疗,提高脐尿管肿瘤的诊治效果.方法:回顾性总结2005年-2016年收治的15例脐尿管病变的诊治过程.结果:经超声、CT、膀胱镜及病理等检查,确诊脐尿管腺癌11例,2例行根治性膀胱切除术,2例行膀胱部分切除术,11例行扩大性膀胱部分切除术.术后病理回报:黏液腺癌11例,印戒细胞癌1例,脐尿管瘘1例,子宫内膜异位症1例,脐尿管囊肿1例.4例行单一化疗,7例行联合化疗.术后均获随访.结论:超声、CT及膀胱镜等检查能及时发现脐尿管病变,扩大性膀胱部分切除术可作为脐尿管腺癌的主要手术方式.

An analysis of 15 cases with urachal lesions

Objective:To improve the diagnosis and theatment through early detection and treatment of urachal tumor. Methods:a retrospective study of 15 cases of urachal lesions diagnosis and treatment from 2005 to 2016 were summarized. Results:Ultrasonography,CT,cystoscopy and pathology were performed in 11 cases. Ureteral adenocarci-noma was diagnosed in 11 cases,2 cases underwent radical cystectomy,2 cases underwent partial cystectomy and 11 cases underwent enlarged partial cystectomy. Pathological findings were mucinous adenocarcinoma in 11 cases,signet ring cell carcinoma in 1 case,urachial fistula in 1 case,endometriosis in 1 case,and urachal cyst in 1 case. 4 cases were treated through single chemotherapy,7 cases of combined chemotherapy. All patients were followed up after sur-gery. Conclusion:Urachal lesions could be observed timely through ultrasonography,CT,cystoscopy and so on. Ex-tended partial cystectomy could be the main surgical method for urachal tumor.
