支气管肺泡灌洗液联合TCT在周围型肺癌诊断中的应用

目的:探讨支气管肺泡灌洗液联合TCT在周围型肺癌诊断中的应用价值.方法:纳入2014年12月至2015年12月收治的123例周围型肺癌患者,所有患者支气管镜检查未发现病灶,遂同时获得支气管肺泡灌洗液和支气管镜刷片标本,同时采用TCT和直接涂片法检测,比较四种方法的敏感度.并对支气管镜检查结果与术后病理结果进行比较,分析支气管镜检查结果的特异性.结果:支气管肺泡灌洗液联合TCT的敏感度显著高于支气管肺泡灌洗液直接涂片法检测的敏感度(36.6% vs 17.9%,P<0.05);支气管镜刷片联合TCT的敏感度亦显著高于支气管镜刷片直接涂片法检测的敏感度(14.6% vs 8.1%,P<0.05);而且四种方法中支气管肺泡灌洗液联合TCT的敏感度最高(P<0.05).支气管镜检查结果与术后手术组织病理结果比较的特异性为100%.支气管肺泡灌洗液联合TCT的分类诊断和手术病理组织学诊断的符合率为84.4%.结论:支气管肺泡灌洗液联合TCT能提高周围型肺癌的检出诊断率,支气管肺泡灌洗液联合TCT可推广应用于临床.

Application of bronchoalveolar lavage fluid combined with ThinPrep cytology test in the diagnosis of peripheral lung cancer

Objective:To investigate the value of bronchoalveolar lavage fluid combined with ThinPrep cytology test(TCT) in the diagnosis of peripheral lung cancer.Methods:The data of one hundred and twenty-three patients diagnosed with peripheral lung cancer were recruited from December 2014 to December 2015.No lesions had been found in bronchoscope examinations of all patients.Therefore,bronchoalveolar lavage fluid and blind bronchial brushing specimen were been obtained in all patients.TCT and direct smear assay were conducted for all specimens.The sensitivity of four cytology methods was compared.The specificity of bronchoscopy cytology was analyzed.Results:The sensitivity of bronchoalveolar lavage fluid combined with TCT was significantly higher than that of bronchoalveolar lavage fluid direct smear assay(36.6% vs 17.9%,P<0.05),while the sensitivity of blind bronchial brushing combined with TCT was significantly higher than that of blind bronchial brushing direct smear assay(14.6% vs 8.1%,P<0.05).Moreover,the sensitivity of bronchoalveolar lavage fluid was higher than the other 3 methods(P<0.05).The specificity of bronchoscopy cytology were 100%.The coincidence of bronchoalveolar lavage fluid combined with TCT and surgical pathology were 84.4%.Conclusion:Bronchoalveolar lavage fluid combined with TCT is more valuable in the diagnosis of peripheral lung cancer and can be widely used in clinical.
