摘    要:目的:根据美国临床实验室标准化委员会(NCCLS)EP9-A2文件,对罗氏DPP生化检测系统(以下简称DPP系统)和美国强生Vitros-350干化学分析系统(以下简称350系统)进行比对试验,为本实验室不同检验系统检测结果的-致性提供实验依据。方法:以DPP系统为参考系统,以350系统为试验系统,对患者样本的钾离子(K+)、钠离子(Na+)、氯离子(CL-)、二氧化碳结合力(C02-cp)、肌酐(Cr)、血糖(Glu)和淀粉酶(AMY)7个较常用的急诊项目进行方法学比对、相关性分析并判断偏倚是否可以接受。结果:K+、Na+、CL-、C02-cp、Cr、Glu、AMY七个检测项目的r值均大于0.975,有很好的相关性;Cr在低Xc和Glu在高xc时两系统间的偏差超过1/2Ea,为临床不可接受,其余项目偏差均在临床可接受范围。结论:当实验室使用两种以上的检测系统检测同-项目时,应进行方法比对和偏差评估,判断其临床可接受性,以保证检验结果的可比性。

Abstract:Objective : To proceed comparison experiment on the wet type analyzer (Roche DPP)and the dry type analyzer(Vitros-350),according to the Document EP9-A2 of the National Committee for Clinical Laboratory Standards(NCCLS)and to provide experimental basis for the consistency of the laboratory testing results of different test systems. Methods : Use Roche DPP as reference system and Vitros-350 as testing system respectively to determine seven emergency items ( K+, Na+. CL-, CO2-cp Cr , Glu and AMY ) in all then compare the methodology and evaluated the bias within methodological linear limitation.Results : K+, Na+, CL-, CO2-cp, Cr, Glu and AMY of the two different biochemical analyzers have good correlation (r〉0.975); the bias of Cr when in low Xc and Glu when in high Xc are morn than 1/2Ea which is unacceptable, meanwhile the other bias are acceptable. Conclusions : If the same items are tested by more than two systems, it' s necessary to judge clinical acceptability by method comparison and bias estimation so as to insure the comparability.
