乳腺癌中雌、孕激素受体和p53表达的临床意义

摘    要:目的:探讨乳腺癌组织中雌激素受体(ER)孕激素受体(PR)和p53的表达与乳腺癌临床分期、腋淋巴结转移和预后的关系。方法:采用链霉菌抗生物素蛋白挝氧化物酶链接法(SP法)检测96例乳腺癌组织和10例乳腺良性增生病变组织中ER、PR和p53的表达,并对不同临床分期和有无腋淋巴结转移乳腺癌组织中ER、PR和p53的阳性表达率进行比较。结果:乳腺癌组织中ER、PR和p53阳性表达率分别为60.4%、62.5%、59.4%,乳腺良性增生病变组织中ER、PR和p53阳性表达率分别为80.0%、80.0%、0,乳腺良性增生病变组织和乳腺癌组织中ER、PR阳性表达率差异无统计学意义(P〉0.05),而p53的阳性表达率差异有统计学意义(P〈0.05);不同临床分期乳腺癌组织中ER和PR的阳性表达率差异无统计学意义(P〉0.05),随着临床分期的增加,p53的阳性表达率明显升高(P〈0.05);无腋淋巴结转移者ER和PR的阳性表达率明显高于有腋淋巴结转移者(P〈0.05),p53的阳性表达率明显低于有转移者(P〈0.05)。结论:联合检测乳腺癌组织ER、PR和p53的表达对于判断乳腺癌预后和指导治疗具有临床价值。

Abstract:Objective: To investigate the expression of estrogen receptor(ER),progesterone receptor(PR) and p53 in the primary breast carcinoma and the relationship among the expression and the clinical stage,axillary lymph node metastasis and prognosis of the patients.Methods: The expression of ER,PR and p53 in 96 paraffin-embeded breast carcinoma and 10 hyperplastic disease of the breast tissues were studied with immunohistochemical technique(streptavidin biotin peroxidase,SP).To compare the positive rate of ER,PR and p53 in breast carcinoma with clinical stage,axillary lymph nodes metastasis.Results: The positive rate of expression of ER,PR and p53 in breast carcinoma were 60.4%,62.5%,59.4% respectively.No significant difference was found in the expression of ER,PR between hyperplastic diease of the breast and primary breast cancer(P>0.05),while the difference of p53 expression was seen significantly between two groups(P<0.05).There was not significant difference between expression of ER,PR and clinical stage(P>0.05).But the expression of p53 in primary breast cancer increased remarkably as the clinical stage growing(P<0.05).The expression of p53 in patients with axillary lymph node metastasis was significantly higher than that of without(P<0.05),while ER and PR expression had the inverse result(P<0.05).Conclusion: Combined detection of expression of ER,PR and p53 in primary breast carcinoma has practical value in prognosis of the patients and therapeutic effects.
