试论卫生改革中的“公平优先”原则

文章通过对我国卫生服务体制发展过程的回顾,结合目前的宏观社会经济环境,提出了卫生服务公平性的问题在我国的形成。通过对公平和效率概念的解释和两者辩证关系的阐述,作者认为我国卫生服务体制改革应遵循“公平优先”原则。并对其必要性加以论述,认为“公平优先”原则是卫生事业特殊性的要求,是健全和完善社会保障体制的需要。“公平优先”原则的现实意义表现在,有利于社会的稳定与发展,有利于提高居民健康水平、加快经济发展,有利于改善党和政府在人民群众心目中的形象。同时,强调公平优先,不是不要效率,应该注重长远效率的提高。

Discussion on the principle of equity priority in health reform in China

Reviewed the development of health service system in China, combined with macro-socio-economic environment, the article described the formation of inequity in health service. Explained the concepts of equity and efficiency and their dialectic relationship, authors think reform of health service in China should keep the "principle of equity priority". And authors also discussed the necessity of this principle, pointed out that it's determined by the particularity of health enterprise, and it's also the need of social security system perfection. Practical significance of the principle is shown in the following aspects. It would be in favor of social stabilization and development, good for enhancing people's health status and fastening economy increase and propitious to improve figures of the Party and Government among Chinese people. However, emphasis of equity doesn't mean to abandon efficiency. We should pay more attention to increase of long-term efficiency.
