腹腔镜腹膜外途径前列腺癌根治性切除术的初步探讨

目的探讨经腹膜外途径腹腔镜前列腺癌根治性切除术治疗前列腺癌的方法。方法对3例前列腺癌患者实施了腹腔镜前列腺根治性切除术。术前3例患者前列腺活检均确诊为前列腺癌,Gleason肿瘤分级分别为：2＋3、2＋3、3＋4;TNM分期T2b。结果 3例手术均顺利完成,手术时间分别为240、260、225min,平均手术时间为242 min,术中均未输血,未出现并发症。术后病理检查均确诊为前列腺癌,切缘阴性。术后3-4d拔引流管,14d拔除导尿管,其中2例发生轻度尿失禁,经加强缩肛动作等锻炼,4-9个月后逐步恢复自主排尿功能。术后3个月均未见生化复发。随访2-10个月,未发现局部复发和远处转移。结论腹膜外途径腹腔镜前列腺癌根治性切除术是一种安全有效的手术方法,同传统的开放性手术比较,具有创伤小、恢复快等优点。但其远期效果还需进一步观察研究。

An evaluation of laparoscopic radical prostatectomy in patients with prostate cancer
