摘    要: 目的研究红细胞输注对急性颅脑创伤患者围术期脑氧供需平衡的影响.方法随机选择40例急性闭合性颅脑损伤行开颅探查患者,对照组20例,输入全血;试验组20例,输入红细胞.监测患者围术期颈静脉球及桡动脉血气,评价其对脑氧供需平衡的影响.结果红细胞输入组术毕脑氧摄取率(CEO2)和动脉与颈静脉球氧含量差[C(a-jv)]O2与对照组相比无明显变化(P>0.05).结论在维持血流动力学稳定状况下,单纯红细胞输入不影响急性颅脑损伤患者围术期脑氧供需平衡.

Abstract:Objective To study the effect of erythrocyte infusion on the cerebral oxygen supply and consumption in patients with cranio-cerebral frauma during perioperative period. Methods Forty patients with severe head injury undergoing inhalation anesthesia and craniotomy were randomly assigned to two groups: Control group ( n = 20) was given infusion of whole blood whereas treatment group ( n ?20)red blood cells.Their blood gas in blood samples taken from radial artery and bulb of jugular vein was monitored perioperatively. Cerebral oxygen extraction (CEO2) and concentration of cerebral arteriovenous oxygen difference[C(a-jv)O2] were calculated. Results CEC^ and C(a-jv)O2 in treatment group did not change significantly as compared with those in control group ( P > 0.05). Conclusion Infusion of red blood cells does not change the balance of cerebral oxygen supply and consumption in patients with severe head injury perioperatively.
