静脉注射合贝爽与硝酸甘油治疗不稳定性心绞痛临床疗效比较

目的　探讨合贝爽与硝酸甘油静脉注射治疗不稳性心绞痛的疗效。方法　选择 86例不稳定性心绞痛病例 ,随机分成两组 ,每组 4 3例。观察组应用合贝爽于开始 5min内给负荷量 2 0mg ,之后按 5mg/h速度静点 ,如无不良反应 ,10min内加量至 15mg/h持续静点 ,硝酸甘油组给硝酸甘油 1mg/h速度静点 ,如无不良反应 10min内加量达 3mg/h静点。观察用药前后心绞痛次数、持续时间、血压和相关临床症状变化。结果　合贝爽组在 4 8h前心绞痛发作次数为 (6 1± 2 3)次 ,用药后为 (1 1± 0 5 )次 ,持续时间用药前后分别为 (16 7± 7 7)min ,(10 7± 5 7)min。硝酸甘油组发作次数用药前后 (5 9± 2 7)次 ,(2 3± 0 7)次 ,持续时间用药前后分别为 (17 1± 5 8)min ,(13 7± 3 6 )min。血压和心率 ,用药后明显下降 ,而用硝酸甘油组明显升高。结论　合贝爽治疗不稳定性心绞痛疗效确切、安全 ,对减轻心肌缺血更有效

Comparison of Curative Effects of Treatment of Unstable Angina with Hebeishuang Injection and Nitroglycerine

Objective To discuss the curative effect of treatment of unstable angina (UA) with Hebeishuang injection and nitroglycerine. Methods 86 cases of UA patient were divided into 2 groups (43 cases per each). The test group was treated with Hebeishuang at 5 mg/h i.v. after first dosage of 20 mg within 10 min. The 15 mg/h i.v. was maintained if no side effect. The control group was treated with nitroglycerine 3 mg/h i.v. after 1 mg/h within 10 min. The incidence of angina, duration, Bp and relative clinical syndrome before and after treatment were recorded. Results The attack times was 6.1±2.3 before angina 48 h, 1.1±0.5 after treatment; the duration of medication was 16.7±7.7 min before treatment, 10.7±5.7 min after treatment in test group. The attack times was 5.9±2.7 before angina 48 h, 2.3±0.7 after treatment; the duration of medication was 17.1±5.8 min before treatment, 13.7±3.6 min after treatment in control group. The Bp and HR were decreased in test group and increased in control group. Conclusion The Hebeishuang injection might reduce the ischemia of myocardium and treat UA effective and safe.
