H-FABP在内毒素致大鼠心肌损伤中的变化及意义

摘    要:目的 研究心型脂肪酸结合蛋白在内毒素致大鼠心肌损伤中的动态变化及其规律,为临床脓毒症心肌损伤的诊断及心功能评估提供动物实验的依据。方法 将雄性 Wistar 大鼠随机分组:正常对照组、内毒素组。内毒素组:腹腔注射 LPS(10mg/kg,用 2mlNS 溶解)后,正常对照组:腹腔注射同等剂量生理盐水。于3、6、12、24、48h眼球后静脉丛取血,测量血清中心型脂肪酸结合蛋白(H-FABP)、肌钙蛋白 T(cTnT)、心型肌酸激酶(CK-MB)、脑钠肽(BNP)含量。结果 ①H-FABP:与正常对照组相比,内毒素各组的 H-FABP 含量明显升高,其中 H-FABP 含量在 3h 升高,6h 达到高峰,12h 出现快速下降,48h 下降幅度减少(P<0.01);②cTnT:与正常对照组相比,cTnT 在6h 明显升高,12h 达到高峰,24h 有所下降,48h 仍保持较高浓度(P<0.01);③CK-MB:与正常对照组相比,CK-MB 在6h 明显升高,12h 达到高峰,48h 保持在较高浓度(P<0.01);④BNP:与正常对照组相比,内毒素各组 BNP 明显升高,并随时间推移,呈上升趋势(P<0.01)。结论 血清H-FABP可作为诊断内毒素心肌损伤的早期生物标记物之一,并与 cTnT 和CK-MB 相结合,能更好的发现内毒素的心肌损伤及严重程度。

Changes and significance of h-fabp in myocardial injury induced by LPS in rats

Abstract:Objective To study the dynamic changes and rules of cardiac injury induced by endotoxin in rats, so as to provide experimental basis for the diagnosis of myocardial injury in clinical sepsis and the evaluation of cardiac function.Methods 24 male Wistar rats were randomly divided into normal control group (n=12) and endotoxin group (n=12).In the endotoxin group, LPS (10mg/kg, dissolved with 2mlNS) was intraperitoneal injection. In the normal control group, the same dose of normal saline was intraperitoneal injection.At 3, 6, 12, 24, and 48h, blood was collected from the posterior venous clumps of the eyeball, and serum levels of central fatty acid-binding protein (h-fabp), troponin T (cTnT), cardiac creatine kinase (ck-mb), and brain natriuretic peptide (BNP) were measured.Results H-FABP: compared with the normal control group, the content of h-fabp in the endotoxin group increased significantly. The content of h-fabp increased at 3h, peaked at 6h, decreased rapidly at 12h, and decreased at 48h (P<0.01).CTnT: compared with the normal control group,cTnT significantly increased at 6h, peaked at 12h, decreased at 24h, and remained relatively high at 48h (P<0.01).Ck-mb: compared with the normal control group, ck-mb significantly increased at 6h, reached the peak at 12h, and remained at a relatively high concentration at 48h (P<0.01).(4) BNP: compared with the normal control group, BNP in the endotoxin group increased significantly, and showed an increasing trend with the passage of time (P<0.01).Conclusion serum h-fabp can be used as one of the early biomarkers for the diagnosis of endotoxin myocardial injury, and can be combined with cTnT and ck-mb to better detect the myocardial injury and severity of endotoxin.
