肺部超声联合心脏超声在小儿急性呼吸窘迫综合征中的诊断价值

摘    要:目的:探讨肺部超声联合心脏超声在小儿急性呼吸窘迫综合征(PARDS)中的诊断价值。方法:纳入2018年7月~2020年7月我院收治的PARDS患者98例,随机分2组,对照组49例,均接受单纯肺部超声诊断,研究组49例,均基于肺部超声,联合心脏超声诊断,以2015年儿童急性肺损伤委员会(PALICC)制定的PARDS诊断标准为金标准,比较两组诊断结果。结果:研究组病情诊断结果与金标准诊断结果的符合率为97.96%,较对照组85.71%明显更高(P<0.05),研究组对PARDS预后预测的灵敏度(85.71%)、特异度(92.86%)较对照组(灵敏度62.86%、特异度57.14%)均明显更高(P<0.05)。结论:基于肺部超声,在PARDS临床诊断中联用心脏超声具有较高应用价值,可明显提高患者病情及预后预测的准确性。

Abstract:Objective: To explore the diagnostic value of lung ultrasound combined with cardiac ultrasound in children with acute respiratory distress syndrome (PARDS). Methods: A total of 98 PARDS patients admitted to our hospital from July 2018 to July 2020 were enrolled. They were randomly divided into 2 groups. 49 cases in the control group were diagnosed with pure lung ultrasound. The 49 cases in the study group were all based on lung ultrasound. Combined with cardiac ultrasound diagnosis, using the PARDS diagnostic criteria established by the Children"s Acute Lung Injury Committee (PALICC) in 2015 as the gold standard, the diagnostic results of the two groups were compared.Results: The coincidence rate between the study group"s disease diagnosis results and the gold standard diagnosis results was 97.96%, which was significantly higher than that of the control group 85.71% (P<0.05). The study group had a sensitivity (85.71%) and specificity (92.86) for predicting the prognosis of PARDS. %) were significantly higher than the control group (sensitivity 62.86%, specificity 57.14%) (P<0.05). Conclusion: Based on pulmonary ultrasound, the combined use of cardiac ultrasound in the clinical diagnosis of PARDS has a high application value, which can significantly improve the accuracy of patient condition and prognosis prediction.
