摘    要:目的应用右胸前外侧小切口,心脏不停跳完成小儿心内直视手术,以减轻手术对病儿的身心创伤,同时达到美容效果。方法全组共16例患儿,其中单纯继发孔型房缺12例,房缺合并部分型肺静脉异位引流4例,均采用经肋间小切口,在常温体外循环辅助、心脏不停跳下完成直视手术。结果手术顺利,无死亡及相关并发症发生。结论对传统上经右心房切口入路的心内直视手术,经右胸壁微创切口、常温心脏跳动中完成,具有创伤轻、手术中无心肌缺血缺氧和再灌注损害、体外循环时间短、病儿术后恢复顺利和美容效果等优点。

Abstract:Objective The Heart-beating open heart surgery and the minimally invasive right anterolateral incision technique were introduced to reduce the psycosomatic injuries of the common CPB surgery to the patients and satisfy patients demands for comestic effects. Method Sixteen children with common congenital heart disease(CHD),were chosen to have heart-beating open heart surgery performed with cardiopulmonary bypass supporting under normal temperature through right anterolateral intercostal minimally invasive incision. Result The operation was successful without death and corresponding complication. ConclusionThe open heart surgery traditionally through right atrium incision could be done throngh right anterolateral minimally invasive incision under normal temperature with beating heart,The techniques have such advantages as smaller incision,no reperfusion injury,reduced CPB time,less traumatic damages and cosmetic effects.
