| 摘 要： | 目的探讨泛影葡胺在粘连性肠梗阻的诊断和治疗中的效果及手术时机的选择。方法对114例粘连性肠梗阻患者经胃管注入76%泛影葡胺50～100 mL,行胃肠道造影,通过腹部立位平片动态观察泛影葡胺在肠管中的位置及通过情况,以确定梗阻部位,以及梗阻是否完全,从而确定手术时机。结果114例中,92例造影剂在7～48 h后到达结肠而给予保守治疗,另22例泛影葡胺在48 h后不能到达结肠,且症状体征无改善或加重而行手术治疗,均痊愈出院。结论泛影葡胺肠管造影在粘连性肠梗阻的诊治中,对明确梗阻部位及手术时机的选择方面有很好的指导作用,同时其还具有治疗作用,可明显降低粘连性肠梗阻患者的手术率和死亡率。

| Abstract: | Objective To discuss the effect of urografin in diagnosis and treatment of adhesive intestinal obstruction.Methods 114 cases of such patient were injected with urografin through gastric tube for 50～100 mL.In standing position,continuous observation was taken to monitor the urografin position in intestine so that to obtain the location of obstruction and degree.Results There were 92 cases who underwent conservative therapy after 7～48 h urografin study to colon.There were 22 cases underwent operation after 48 h whose symptom and sign had no relief or improved.All cases were discharged after cured.Conclusion This method might have indicative function in diagnosis of location and degree and treatment that might also decrease mortality of operation. |
