结核性支气管狭窄61例的CT表现

目的 提高对结核性支气管狭窄CT表现的认识。资料与方法 回顾性分析61例结核性支气管狭窄的CT扫描资料。为了比较，同时复习33例经病理证实的中心型肺癌CT片。结果 CT片上，61例共显示狭窄支气管128支，包括右侧72支(56．3％)和左侧56支(43．7％)。其中37例(60．7％)显示2支以上的狭窄支气管，15例(24．6％)主、叶、段多支支气管同时受累。支气管狭窄表现为向心性管状狭窄38例(62．3％)、漏斗状狭窄15例(24．6％)和闭塞8例(13．1％)。支气管狭窄程度表现为轻度34例(55．7％)、中度19例(31．2％)和重度8例(13．1％)。狭窄腔边缘显示不规则30例(49．2％)，光滑23例(37．7％)。结论 轻或中度向心性管状或漏斗状狭窄，狭窄段较长或主、叶、段多支支气管受累是结核性支气管狭窄常见的CT表现。

CT Manifestations of Tuberculous Bronchial Stenosis:A Report of 61 Cases

Objective To make a further understanding of the CT signs of tuberculous bronchial stenosis.Materials and Methods CT manifestations in 61 patients with tuberculous bronchial stenosis were retrospectively analyzed. CT findings in 33 cases with pathologically proved central pulmonary cancer were also reviewed. The results were compared.Results On CT scans, 128 stenosed bronchi were displayed, which included 72 (56.3%) bronchi on the right side and 56 (43.7%) bronchi on the left side. Thirty-seven patients had 2 or more stenosed bronchi. Involvement of multiple bronchi, including main, lobar and segmental bronchi, was found in 15 cases (24.6%). Bronchial stenosis was presented as concentric tubular stricture (n=38, 62.3%), funnel-shaped stricture (n=15, 24.6%) or obliteration (n=8, 13.1%). The stricture degree was mild in 34 (55.7%), moderate in 19 (31.2%) and severe in 8 (13.1%) cases. The border of stenosed lumen was irregular in 30 (49.2%) and smooth in 23 (37.7%) cases. Conclusion The main CT sign of tuberculous bronchial stenosis is mild to moderate concentric tubular or funnel-shaped bronchial stricture, which involves a long segment of bronchus or several bronchi.
