溃疡病伴同胃炎与胆汁酸和幽门螺旋杆菌的关系

应用组织病理学、胆酸测定和电镜观察三种方法对溃疡病和溃疡病术后残胃炎患者的胃粘膜组织和胃液进行研究。溃疡病患者胃窦粘膜均合并不同程度的炎症，吸窦粘膜幽门螺旋杆菌的检出率为81．97％。比门螺旋杆菌阴性组的胃窦粘膜各类炎细胞计数明显高于阴性组；在幽门螺旋菌感染的胃窦粘膜中，胃小凹增生和腺体萎缩明显多见。残胃炎患者空腹胃液的pH，胆酸浓度显著高于溃疡病者，而幽门螺旋杆菌检出率则较后者低。电镜下显示幽门螺旋杆菌对其粘附的微绒毛有破坏作用。

THE ASSOCIATION OF HELICOBACTER PYLORI BILE ACID AND ACCOMPANYING GASTRITIS IN ULCER DISEASE

The gastric mucosal tissue and gastric fluid in patients with ulcer disease and stump gastritis after operation for peptic ulcer were studied with histopathology,detec- tion of bile acid and electron microscope observation.Histologic examination shows that gastric antral gastritis was seen in all patients with ulcer disease.In 81.97% patients with ul- cer disease,Helicobacter pylori(Hp) was detected in gastric antral mucosa.The scores for all classes of inflammatory cells in antral mucosa in patients with ulcer disease were significantly higher in the Hp positive patients than the Hp negative ones.Faveolar hyperplasia and glandular atrophy were more common in the antral mucosal infected with Hp.Compared with the ulcer patients,fasting bile acid concentration and pH in stump gastritis patient were obviously higher,and positive rates of Hp was lower.It was investi- gated that Hp disrupted mierovilli to which they adhere directly.
