椎体结核的MRI诊断与鉴别

摘    要:目的: 探讨椎体结核的MRI表现并与椎体肿瘤及转移瘤相鉴别。材料和方法: 26 例经手术证实的椎体结核及11 例临床证实的椎体肿瘤及转移瘤, 对其MRI表现进行观察分析。结果: 椎体结核椎间盘均破坏变窄, 椎体破坏部均与间盘相邻。椎体与间盘均有破坏的20 例伴有椎旁冷脓肿。10 例脊柱转移瘤及1 例椎体原发肿瘤间盘未受累。结论: 椎体结核有其特征性表现, 仔细观察椎体受累的形态, 间盘及附件有无破坏以及冷脓肿的形成对椎体结核的诊断及鉴别诊断非常有益。

Abstract:Objective:To probe the MRI features of tuberculosis of vertebral body and differential diagnosis from bone tumor and metastasis.Materials and Methods:26 cases were surgical proved tuberculosis of vertebral body were selected for investigation.Results:In tuberculosis of vertebral body the inter-vertebral discs were destroyed and narrowed,destruction of the vertebral body occured adjacent to the disc.Abscess formation was seen in 20 cases where there were destruction of vertebral body and intervertebral disc.There are no involvement of the intervertebral disc in metastasis and tumor of vertebral body.Conclusion:The tuberculosis and tumors of vertebral body demonstrated their special MRI features,careful observation of the shape of the vertebral body involved,any involvement of the disc and accanory parts,presence of abscess formation was helpful in differential diagnosis of tuberculosis and tumor of vertebral body.
