盆腔自主神经保留对直肠癌全直肠系膜切除术男性患者排尿及性功能的影晌

【目的】探讨盆腔自主神经保留（PANP）在直肠癌全直肠系膜切除术（TME）中的应用对男性排尿及性功能的影响。【方法】将本院60例接受直肠癌手术的男性患者随机分成两组。观察组行PANP＋TME手术，对照组仅行TME手术。比较两组患者术后排尿和性功能障碍的发生率。【结果】观察组患者术后排尿功能障碍的发生率为30％（9／30），明显低于对照组80％（24／30）；观察组术后勃起功能和射精功能障碍的发生率分别为26．7％（8／30）和30％（9／30），也明显低于对照组76．7％（23／30）和83．3％（25／30），且两组相比较差异有显著性（P〈0．05）。两组局部复发率均为6．7％（2／30），两组相比较差异无显著性（P〉0．05）。【结论】在保证根治的前提下，盆腔自主神经保留能有效降低患者术后排尿功能障碍和性功能障碍的发生率，提高术后生存质量。

Application of Pelvic Autonomic Nerve Preserving in Total Mesorectal Excision for Rectal Cancer

][Objective] To explore the application of pelvic autonomic nerve preserving（PANP） in total meso rectal excision（TME） for rectal cancer and its effect on urinary and sexual function of male patients. [Methods] Sixty male patients undergoing the surgery of rectal cancer in our hospital were randomly divided into two groups. The observation group underwent PANP and TME, while the control group only underwent TME. The incidence of sexual dysfunction and urination disorder rate after operation were compared between two groups. [Results] The incidence of urination disorder in observation group after operation was 30 % （9/30） which was obviously lower than that in control group（80%, 24/30）. The incidence of erection disorder and ejaculation disorder in observation group after operation were 26.7% （8/30） and 30% （9/30） respectively, which were obviously lower than those in controlgroup（76.7%, 23/30 and 83.3%, 25/30, respectively）, and there were significant differences between two groups（ P 〈0.05）. The recurrence rate of both groups was 6.7% （2/30）, but there was no significant differ ence between two groups（ P〉0.05）. [Conclusion] On the premise of radical cure,. PANP can effectively reduce the incidence of urinary and sexual dysfunction and improve the life of quality after operation.
