ITP患者促血小板生成素与T淋巴细胞亚群的相关性分析

摘    要:【目的】探讨免疫性血小板减少性紫癜(ITP)患者促血小板生成素(TP0)与T淋巴细胞亚群的相关性。【方法】采用大剂量地塞米松治疗ITP患者(治疗组),根据疗效分为有效组和无效组,同时收集健康志愿者(对照组),检测各组外周血循环中TPO、血小板巨核细胞数量及T淋巴细胞亚群,并分析TPO与T淋巴细胞亚群的相关性。【结果】治疗组中有效组TP0明显低于无效组(P〈0.05);相关分析表明激素治疗有效组患者血清TPO水平与PLT、巨核细胞数呈负相关;与正常对照组相比,ITP组CD3+和CD4+明显下降,CD8+明显升高,CD4+/CD8+比例明显下降,ITP激素治疗有效组T淋巴细胞亚群变化与正常对照组比较差异无显著性,CD4+、CD8+、CD3+的含量与PLT存在相关性;激素治疗有效患者血清TPO和血小板水平与CD4+、CD8+、CD3+舍量存在相关性。【结论】ITP患者TPO水平与T淋巴细胞亚群的变化有关。

Abstract:[Objective] To explore the correlation between thrombopoietin(TPO) and T-lymphocyte subsets in patients with immunologic tbrombocytopenic purpura(ITP). [Methods] ITP patients(treatment group) received high dose dexamethasone(HD-DXM) and were divided into effective subgroup and ineffective sub-group according to the efficacy. Meanwhile, healthy volunteers(control group) were collected. TPO in peripheral blood, the number of platelet(PLT) and megakaryocytes and T-lymphocyte subsets in all groups were detected. The correlation between TPO and T-lymphocyte subsets was analyzed. [Results]TPO in effective sub- group of treatment group was obviously lower than that in ineffective subgroup( P 〈0.05). The correlation a and megakaryocytes. Compared with normal control group, CD3+ and CD4+ in ITP group were decreased obviously, and CD8+ was increased obviously, and CD4+/CD8+ ratio was decreased obviously. There was no significant difference in the change of T-lymphocyte subsets between hormone effective subgroup and normal control group. The levels of CD4+ , CD8+ and CD3+ were associated with PLT. Serum TPO and PLT in hormone effective subgroup were associated with the levels of CD4+ , CD8+ and CD3+. [Conclusion] TPO level in ITP patients is associated with the change of T-lymphocyte subsets.
