电生理监测在颅内动脉瘤夹闭术中的应用

摘    要:【目的】探讨躯体感觉诱发电位(SEP)和脑电图(EEG)监测在颅内动脉瘤夹闭术中的应用价值。【方法130例颅内动脉瘤在手术夹闭中持续进行SEP和EEG监测,对电生理信号异常者及时采取干预措施,并对其疗效进行分析。【结果]30例患者中11例术中电生理信号无异常改变,14例电生理信号异常,经干预后恢复正常,术后CT均未出现新的缺血性改变、神经功能良好。5例异常者经干预后未恢复正常,术后CT出现新的缺血性改变、并有新的神经功能缺失。【结论】动脉瘤夹闭术中持续监测SEP、EEG有助于及早发现和干预手术操作引起的脑缺血,对减少手术并发症有重要作用。

Abstract:[Objective] To explore the value of somatosensory evoked potentials(SEP) and electroencephalogram(EEG) monitoring used in the clipping of intracranial aneurysm. [Methods]SEP and EEG were continuously monitored in 30 patients undergoing the clipping of intracranial aneurysm on 30 patients. Intervention measures were taken timely for patients with abnormal electrophysiological signals. The efficacy was analyzed. [Results] Among 30 patients, 11 patients had no change of abnormal electrophysiological signals during the operation, and 14 patients had abnormal electrophysiological signals, and were recovered after intervention, and showed no new ischemic changes by CT and no neurological deficits after the operation. Other 5 patients with abnormal electro- physiological signals were not recovered after intervention, and showed new ischemic changes by CT, and devel oped functional deficits after the operation. [Conclusion]Continuous monitoring of SEP and EEG during the clipping of intracranial aneurysm is helpful for early finding and timely intervening cerebral ischemia caused by operative procedure, and plays an important role in reducing the surgical complications.
