高龄患者腹腔镜胆囊切除术并发症的预防

目的总结75岁以上高龄患者腹腔镜胆囊切除术（Lc）的经验和教训，探讨提高成功率和预防并发症的措施。方法回顾分析167例75岁以上高龄胆囊结石患者施行LC的临床资料，其中伴急性胆囊炎93例、伴胆总管结石3例。结果LC成功率97．6％，LC中转开腹4例。术后发生并发症18例（10．8％），死亡2例（1．2％）。结论LC术是一种治疗高龄胆囊疾病较为安全有效的方法。由于高龄机体应激能力减退、合并内科疾病多，术后并发症相对偏多，须把握手术时机和灵活运用手术技巧，加强围手术期的处理，才能有效地预防并发症。

The prevention of complications for elderly patients receiving laparoscopic cholecystectomy

Objective The experience and lessons of patients over 75 years old,who receiving laparoscopic cholecystectomy were studied, and the measures to increase thesuccess rate and prevent complications were investigated. Methods The clinical data of 167 patients with gallstone, who all over 75 years old and receiving laparoscopic cholecystectomy were studied retrospectively, including 93 cases suffering from acutecholecystitis and 3cases suffering from common bile duct stones. Results The success rate of laparoscopic cholecystectomy is 97.6%, and 4 cases transiting to laparotomy. Postoperative complications occurred in 18 cases （10.8%） and 2 cases were dead （1.2%）. Conclusion Laparoscopic cholecystectomy is a safe and effective method to treat elderly patients with gallbladder disease. The stress ability is impaired in elderly patients and they are always suffering other medical disease, so the postoperative complication is common. For these reasons we must grasp the timing of surgery, use the surgical techniques flexibly, and strengthen perioperative treatment, in order to prevent complications effectively.
