恶性肿瘤患者PICC置管静脉血栓形成与D-2聚体相关性研究

目的:恶性肿瘤患者本身存在着凝血和纤维蛋白溶解系统(简称凝溶系统)的异常,加之使用PICC置管,上肢深静脉血栓形成的概率将进一步增加。本研究旨在说明恶性肿瘤患者行PICC穿刺静脉血栓形成与D-2聚体的相关性,为PICC置管患者血栓相关安全性提供一定的参考依据。方法:将本科室所收治的接受PICC置管的恶性肿瘤患者进行D-2聚体检测,对比分析D-2聚体正常与异常情况下,上肢深静脉血栓的发生概率。结果:经统计,两组患者性别及年龄无统计学差异。D-2聚体异常升高组的上肢深静脉血栓发生率明显高于正常组。结论:恶性肿瘤患者D-2聚体的异常升高,在进行PICC置管时,将出现更高的静脉血栓的发生率,此时进行血栓相关干预,可以降低上肢DVT的发生率,提高PICC置管的安全性。

Correlative study on the DVT in malignant tumor patients with PICC and D-dimmer

Objective：The rate of DVT in upper limb will increase in malignant cancer patients, because of the abnormal with Coagulation and Fibrinolysis System （CFS）, in addition to the use of PICC.In order to describe the correlation of DVT in malignant cancer patients with PICC and D-dimmer in order to provide the evidence for the safety of patients with PICC.Methods：All the cancer patients with PICC will be inspected the D-dimmer and compared with the rate of DVT in normal and abnormal of the D-dimmer.Results：There is no significant difference in age and sexuality of the two groups. The incidence of abnormal elevation group was significantly higher than that of the normal group.Conclusion： When the D-dimmer is significantly high in malignant cancer patients accepting the PICC, there would be significantly higher incidence of the DVT in upper limb. Intervention to the thrombus can be done to decrease the incidence of DVT and raise the safety of the PICC.
