骨水泥椎体成形对胸腰椎压缩性骨折后脊柱稳定性的影响

摘    要:目的:研究骨水泥椎体成形对胸腰椎压缩性骨折后脊柱稳定性的影响。方法选择我院2011年3月~2013年6月收治的76例胸腰椎压缩性骨折患者作为研究对象,遵照知情同意原则并按照治疗方案分为A、B两组各38例,A组采用AF系统固定治疗,B组采用骨水泥椎体成形术治疗,比较两组手术方案对脊柱稳定性的影响。结果两组患者均手术成功,术前两组脊柱畸形角度差异无统计学意义(P>0.05),术后X线片显示B组的脊柱畸形角度为(6.7±1.2)°、明显小于A组的(9.6±1.5)°,组间差异有统计学意义(P<0.05);两组患者术前神经功能Frankel分值比较差异并无统计学意义(P>0.05),术后A、B两组的Frankel分值均明显改善,分别为(3.8±1.3)、(3.1±1.5)分,以B组改善更为明显,组间差异均有统计学意义(P<0.05)。结论骨水泥椎体成形治疗胸腰椎压缩性骨折临床效果满意,有利于维持脊柱稳定性,是临床首选的手术方案之一。

Abstract:Objective To study the effect of bone cement vertebroplasty on spinal stability after thoracolumbar vertebral compression fractures. Methods 76 cases with thoracolumbar vertebral compression fractures admitted into our hospital from March 2011 to June 2013 were selected as the research objects, which were divided into the group A and the group B in accordance with the principle of informed consent and the treatment program, each group had 38 cases, and the patients in the group A were used AF system fixation treatment, while the patients in the group B were used bone cement vertebroplasty treatment. The effect of two operation schemes on the spinal stability were compared and analyzed. Results The patients of two groups had successful operation, there was not statistical significant difference in the spinal deformity angle before the operation between the two groups(P〉0.05), the postoperative X-ray showed that the spinal deformity angle of the group B was (6.7±1.2)° , which was significantly less than that of the group A[(9.6±1.5) ° ], there was significant difference between the two groups(P 〈 0.05);There was not statistical significant difference in the preoperative nerve function Frankel score between the two groups(P 〉 0.05), the postoperative Frankel score of A and B two groups were significantly improved, respectively (3.8±1.3)score, (3.1±1.5)score, but the improvement of the group B was more obvious, there was significant difference between the two groups (P〈0.05). Conclusion The bone cement vertebroplasty in the treatment of thoracolumbar vertebral compression fractures has satisfactory clinical efficacy, and is conducive to the maintenance of spinal stability, is one of the clinical preferred operation schemes.
