聚维酮碘明胶海绵与胶质银明胶海绵在预防拔牙术后并发症的临床效果比较

目的对比研究聚维酮碘明胶海绵与胶质银明胶海绵在预防拔牙术后疼痛、出血、感染、干槽症等并发症的临床应用效果。方法选择需择期行牙齿拔除术的患者748例793颗牙齿为主要研究对象,以半随机单盲对照方法分为聚维酮碘明胶海绵组（聚维酮碘组）和胶质银明胶海绵组（胶质银组）,均采用局部麻醉,患牙拔除后搔刮干净牙槽窝,聚维酮碘组植入聚维酮碘明胶海绵填塞牙槽窝,胶质组待牙槽窝内血液充塞后填塞胶质银明胶海绵,术后3h、24h、1周复查后观察二组拔牙术后并发症的发生情况。结果聚维酮碘组术后出血症的发生率明显低于胶质银组,比较结果有统计学意义（P〈0.05）;胶质银组术后感染少、肿胀轻,与聚维酮碘组比较结果差异有统计学意义（P〈0.05）;二组患者术后疼痛感及干槽症的发生率比较差异无统计学意义（P〉0.05）。结论聚维酮碘明胶海绵对术后出血治疗效果明显,而胶质银明胶海绵对预防拔牙术后感染及肿胀疗效肯定,临床应用各有千秋,均可推广借鉴。

THE COMPARISON OF IODOFORM GELATIN SPONGE AND COLLOIDAL SLIVER GELATIN SPONGE IN THE PREVENTION OF TOOTH EXTRACTION COMPLICATIONS

ObjectiveTo compare the clinical application effects of iodoform gelatin sponge and colloidal silver gelatin sponge in preventing postoperative pain,bleeding,infection,tooth dry chamfer disease complications.Methods Total 748 patients who needed the elective surgery tooth extraction with 793 teeth were chosen as the main research subjects,and divided into iodoform gelatin sponge group（iodoform group）and colloidal silver gelatin sponge group（colloidal silver group）with the single blind random control method,all the patients were adopted the local anesthetic and cleaned tooth socket after removal,iodoform group implanted with iodoform gelatin sponge tamponade alveolar socket,colloidal silver group packed colloidal silver gelatin sponge after the blood overload in the alveolar socket,the occurrence of two groups of tooth extraction complications were observed at 3h,24h,1 week after the operation.Results The incidence of the disease after iodoform group was significantly lower than that in colloidal silver group（P〈0.05）;Colloidal silver group was with significantly less postoperative infection,swelling compared with iodoform group（P〈0.05）;The incidence of postoperative pain and dry chamfer disease were no significant difference between the two groups（P〉0.05）.ConclusionThere is an obvious effects of iodoform gelatin sponge for the treatment of postoperative bleeding,and colloidal silver gelatin sponge on prevention of postoperative infection and curative effect on swelling of the tooth,the clinical application of each has its strong point,can promote reference.
