小肠浆肌膜腔内自体脾组织移植动物模型的建立与意义

摘    要:目的探讨增加自体脾组织移植量的方法,提高自体移植脾组织的功能。方法贵州小型香猪3头,经肌注麻醉后剖腹切除脾脏,取脾脏的1/2约150g,切成1mm×1mm×1mm小块备用。在距Treitz韧带50cm处,切取保留肠系膜血管的空肠30cm旷置,再吻合肠管恢复肠腔通道,缝闭肠系膜裂口。旷置空肠等分2段,去除肠粘膜,缝闭一端,将约50g脾组织块分别植于2段肠浆肌膜腔内,缝闭另一端口,妥善固定于肠系膜上。大网膜内移植30g脾组织作为对照观察。结果4个月饲养期间内无肠梗阻发。3段肠浆肌膜腔内脾组织再生良好,组织结构与大网膜内移植脾组织无明显差别,2段再生较差,1段形成脓肿。结论肠浆肌膜腔内移植脾组织的量较大,可作为自体脾组织的移植术式。

Splenic autotransplantation to small intestine without mucous membrane in swine

Abstract:Objective To explore more effective measu re for splenic autotranspla ntation to enhance the function of splenic autotransplanted tissue. Met hods Spl enectomy on 3 Guizhou swines was first performed in sterile conditions under gen eral anesthesia by submuscular injection of 3% pentobarbital. A half of the spl e en about 150 g from each animal was cut into pieces (1 mm 1 mm 1 mm) for transpl antation. Two 15-cm long segments of proximal jejunum at the distance of 50 cm f rom Treitz ligment were isolated on their mesentery with intact circulation. Bo w el continuity was re-established by end-to-end anastomosis and the mesentery was sutured. The mucosal layer was removed from the isolated segments of small int e stine and splenic pieces were implanted into the prepared isolated seromuscular loops of small intestine. The ends of the isolated bowel segments were closed. T he bowel segments were fixed to the nearby mesentery to prevent torsion of their vascular pedicles. The splenic pieces about 30 g were implanted in greater ome n tum as control. Results No intestinal obstruction was observe d in 4 months afte r transplantation. Implanted pieces of splenic tissue regenerated well in 3 bow e l segments, not well in 2 bowel segments. An abscess was in 1 bowel segment wit h out regeneration of splenic tissue. Histomorphological appearance of the autotr a nsplanted splenic tissue in the isolated seromuscular loops of small intestine d id not differ from that in greater omentum. Conclusion Isolat ed seromuscular loops of small intestine may be considered as an alternative pla ce of autotransplanted splenic tissue.
