腹盆腔感染性病变的CT及US影像分析

目的探讨腹盆腔化脓性感染与囊肿的CT和US影像特征及其鉴别方法。材料与方法分析25例，男12例，女13例，年龄21～80岁，腹盆腔感染性病变CT和US资料。胰周脓肿5例；阑尾及盆腔脓肿各4例；膈下脓肿3例，前列腺脓肿2例及其他脓肿7例。结果CT征象中包膜不规则增厚、包膜增强及外缘毛糙等对鉴别脓肿和囊肿有意义。US对77.2％病例可明确诊断感染性积液。CT和US对腹盆腔脓肿的检出率分别为100.0％、92.5％。CT的特异性为88.9％。结论CT对定性诊断有很高灵敏度和特异性。US声像图征象结合临床表现有诊断意义。

Imaging Analysis of the Infectious Lesions of Abdominopelvis

Purpose: To analyze CT and ultrasound (US) imaging characteristics of acute pyogenic abscess in the abdomen or pelvic and evaluate two imaging methods for differential diagnosis between pyngenic abscess and noninfectious cysts. Materials and Methods: Twenty-five patients of pyogenic abscesses in the abdominopelvis examined by CT and US were analgzed. There were 12 males and 13 females ranging in age from 21 to 80 years old. The location of these abscesses included peripancreas in 5 patients, appendix in 4, pelvis in 4, prostate in 2, subphren in 3, and others in 7 cases. Results: The features of these abscesses on CT imaging were thickened and irregular capsule, and enhancing rim with irregular peripheral edge after adminstration of contrast medium. US could diagnose 77.2% of the cases of the infectious fluid collections. The sensitivities of CT and US for checking out abscess were 100% and 92.5%, respectively. The specificity of CT was 88.9%. Conclusion: CT has higher sensitivity and specificity for diagnosis of infections lesions of abdominopelvis. US imaging could evaluate infected fluid collection in combination with the clinical matters.
