影响肱骨髁间骨折疗效相关因素分析

目的:评价AO重建钢板固定治疗肱骨髁间骨折的疗效.方法:手术治疗肱骨髁间骨折49例,采用尺骨鹰嘴截骨入路切开复位AO重建钢板内固定.结果:随访12～36个月,平均18个月.术后12～16周骨折全部愈合.采用Aitken-Rorabeck评分系统进行疗效评定优良率为72.6%.伤后9周出现异位骨化1例.结论:肱骨髁间骨折手术前积极止血及消除肌肉水肿,手术解剖复位,手术中严密止血,关节囊重建,术后双重引流,早期进行功能锻炼,将对肱骨髁间骨折愈后疗效起到决定性作用.

Analysis on Relevant Factors of Therapeutic Effect of Humeral Intercondylar Fracture

Objective To evaluate the results of humeral intercondy1ar fracture treated with AO reconstruction internal plate fixation Methods Forty nine patients diagnosed as humeral intercondylar fracture were treated with open reduction and AO reconstruction plate internal fixation by way of olecranon of the ulna osteotomy. The mean follow-up of the group was 18 months ranging 12 to 36 months with bone union in 12 to 16 weeks. Results The outcome was evaluated with Ailken Rorabeck rating system and analyzed. The excellent to good elbow function was 72.6%. A heterotopic ossification developed in one case 9 week after operation. Conclusion Preoperative hemostasis and elimination of muscular swelling, intraoperative anatomical reduction and effective control of bleeding and joint capsule reconstruction,postoperative dual drainage and early function exercises are the key prognostic factors of therapeutic effect of humeral intercondylar fracture.
