| 摘 要： | 目的:分析综合医院重症监护病房呼吸机相关肺炎(ventilator-associated pneumonia,VAP)流行病学特征、相关因素、病原菌分布等特点,为制定预防控制呼吸机相关性肺炎措施提供科学依据.方法:回顾性分析我院2008～2010年重症监护病房发生医院感染VAP 269例临床资料.结果:VAP 232例,占86.24%.VAP相关危险因素为年龄、基础疾病、机械通气时间、侵入性操作、意识障碍等.从插管到发生VAP的时间为2～53 d,平均(8.4±6.3)d.早发性VAP患者(机械通气≤4d)64例(占27.59%),迟发性VAP患者(机械通气>4 d)168例(占72.41%).相比较差异有显著性(P<0.01),VAP病原菌以革兰阴性杆菌为主,占85.01%,以鲍曼不动杆菌、铜绿假单胞菌、肺炎克雷伯菌、大肠埃希菌为主,分别为29.97%,18.65%,15.29%,7.03%.结论:VAP的发生与多种危险因素相关;针对VAP的各种危险因素实施综合干预措施,才能有效的预防和减少VAP的发生.
