逆行前臂桡侧复合蒂筋膜皮瓣的临床应用

目的 总结逆行前臂桡侧复合蒂筋膜皮瓣的临床应用经验.方法 应用逆行前臂桡侧复合蒂筋膜皮瓣29例修复前臂、手部创面,其中以鼻烟壶处为旋转点的皮瓣19例.皮瓣内通常含有头静脉、前臂外侧皮神经、桡神经浅支、桡动脉穿支等复合组织.吻合皮瓣远端神经的有7例.结果 29例皮瓣中27例完全成活,2例皮瓣远端小部分坏死.成活皮瓣质地柔软,血运良好.皮瓣一般比较薄.吻合神经的皮瓣经随访恢复了良好的感觉.结论 前臂桡侧复合蒂筋膜皮瓣血运可靠,切取简捷.远端蒂皮瓣可以修复包含拇、食、中指远端在内的手部较大面积创面.

The clinical application of distally based radial antibrachial composite pedicled fasciocutaneous flaps

Objective To summarize clinical experiece of distally based radial antibrachial composite pedicled fasciocutaneous flaps. Methods 29 distally based radial antibrachial composite pedicled fasciocutaneous flaps were applied to repair the soft tissue defect of the forearm and hand. Among these flaps, the pivot poit of 19 flaps was designed at anatomic snuff-box. These flaps usually contain superficial radial nerve,lateral antibrachial cutaneous nerve,cephalic vein and cutaneous branches of radial artery. Results Twenty-seven of 29 flaps survived completely and the other two had partial necrosis.These flaps have good appearance and abundant blood supply. Conclusions This kind of flap has the merit of high survival rate and easy dissection. The flap can repair the soft tissue defect of distal part of thumb,index finger and middle finger.
